Incompetent and bad attitude

My name is Siyabulela Mqolo. I applied for a cellphone contract at the beginning of November month 2013 to an agent as he took his gracious time to capture the application with excuses that the application is still pending. Only to find out 5 days letter he never capture the request. Until to date I haven't received my cellphone contract. The agent name is Chezlin at Sanlam Head Office in Bellville. This is the second time in one year receiving such service from him as he always with attitude. Every time I'm requesting the information he promised to forward it to me.
