Contract expired in April, still paying

In April 2011 I took a 24 month contract with 8ta which was supposed to end after 24 months i.e. April 2013. This never happened, I never got a call to alert me that this contract was not going to be cancelled, instead 8ta carried on deducting money on my bank account. I realised this in July when I checked my bank statements. I went to 8ta's offices in Greenacres to stop this, I was given a form that I filled in and was notified that July would be the last month that these deductions will take place. I have just received a statement for October indicating that 8ta will be deducting again. This is totally unacceptable as I have cancelled this contract, infact this contract was supposed to have been cancelled end of April already. I contacted 8ta via email to report this and request refund, they responded asking me to download the same form I filled at their offices and asked me to email it to some email address, alternatively, I should go back to their offices to fill in another cancellation form which sounds like a really bad joke. This is totally unacceptable, it is poor service at its best.
