Cancelled contract by paying settlement fee

I signed a contract at rosebank planet fitness in April 2013. In July I moved to Cape Town and cancelled my contract with vitality.
I was told by a staff member at Rosebank Branch that I could get out of my contract by paying at settlement fee of R1800.
I payed the fee in July and was told my contract was cancelled.
Planet fitness have been debiting my account since then with R575 per month as well as a R20 additional fee.
There is no head office number to call so I called the Rosebank brach 6 times only to be told there was no one to speak to and not one person returned my messages.
Eventually I spoke to a lady who said my account was not cancelled after paying meh settlement fee as no one sent me the cancellation form. I was never told about the form and was told after paying the fee the contract would be cancelled.
Now planet fitness owe me the monthly free of R575 for all the months since September 2013.
I have never experienced such bad rude customer service and am struggling to get my money back.
