No assistance with my problem

I was called by Carlene a few days saying I took out a contract with and they want a payment soon. I told her I never applied for an Iphone5 cellphone and I asked to which address was the handset delivered she wouldn't tell me but she knows my identity number. She then requested me to fax her my copy of I'd and an affidavit stating that I never applied which I did but after that I can't get hold of her and I am worried if I get blacklisted for something I didn't receive.
