SIM-Swap Paid for but not processed

After doing my contract upgrade on the 09/08/2013 at the Menlyn iStore I then had to go to the MTN store in Menlyn so I could apply for a SIM swap as the sim card for the iPhone 5 differs from the iPhone 4s. I was helped very quickly and copies of my ID and drivers licence were made by the MTN store. I then signed a document confirming that I required a SIM swap and also received an SMS whilst I was in the Store confirming that a SIM swap has been requested. I then waited until Tuesday (13/08/2013) and nothing happened. When I phoned 808 and asked what was the delay they told me that no SIM swap had been lodged and that I need to return to the MTN store so that they can request it again. The reason I went to do my upgrade on the 9th was because it was a public holiday and I was able to then go as I am very busy during the week. Why after all the sms's and paperwork has my SIM swap now been "lost" in the system and due to this inconvenience now I am sitting with a new phone that can't phone because somebody/something has decided that all the supporting information means absolutely nothing, even if I have all the proof that everything was done and signed?
