Ask to close my account

I am a nursing sister at Medfem Clinic, I paid up my Foschini account and ask them to close the account. I have 3rd degree cancer and want to get all my accounts to be settled. I am not working @ the moment. They never closed the account, so I received a message that they have handed me over to their attorneys. They must have charge me for booklets, which I never received, because my address changed long ago. will you please sort it out for me. The payment was done in February, 13
