Store didnt provide all neccassary documents

Lost my phone which I was under the impression (from purchase reciepts indicating so) that I had insured it.
Upon follow-up of the claim, I discoverered that there were other documents that I should have been given to sign, that I have no knowledge of.

Those documents that had to be signed were to be faxed by the store for the monthly premiums to be deducted. Also to know whether the premiums were deducted or not, was not possible as no email statements are sent to me: as requested by myself when I am always asked for account updating.

I am willing to pay the premiums that were meant to start in month of June 2013 (month after date of purchase: 30th May 2013) as I have known since then to be insured, and its not my fault the store employee (s) didnt complete the neccassary steps of sale.
