Billed for Life Net after my account is settled

For the past few years I have been paying my Foschini account every month. At the end of October 2012 I settled my account in full with Foschini. Because the account was paid in full I did not worry about it any more as I did not owe Foschini any more money, but exactly a month after I paid my last instalment, I receive and invoice stating that I should go and pay R18 for some sort of 'Life net' insurance. At first I thought this was some sort of life insurance someone simply added onto my account without my written consent.

I then went to Foschini in Cresta Shopping centre and asked one of the cashiers what the 'Life net' is. She explained to me that it is an insurance for in case something happened to me and I couldn't pay my account any more. I never gave permission for this to be added to my account.

After numerous phone calls to cancel this and about six months later, it's still not sorted out. Today I received a letter from Foschini stating that I'm being blacklisted. I am very angry about this and feel that it is unfair that my name will now be blacklisted because of a mistake on Foschini's side.

I would appreciate it if someone could respond to me so I can sort this out.
