Money taken after cancellation

Beginning of oct this year we notifed top tv to cancel our debit order and shut down the viewing signal before the next payment which was due 25oct. On several occasions i spoke to some one in the call centre and every time they confirmed our banking details was removed from their system thus no debit order will go off again. On 1 nov a debit order went off for top tv for the amount of R269.00. I phoned them on 2nov when i received notification from my bank of the debit order. At this stage they have several excuses about refunding me. Their service is extremely poor (both for viewing and in the call centre) and we've been having problems for quite a while now where there is no signal and we can't whatch tv. We never skipped a payment. Now that we must be refunded, they are full of excuses. Even said even IF they do refund us, they are going to take a cancellation fee because according to them we never phoned in to cancel.
