Dependant requires disclosure consent form

I am trying to log pre-authorization requests with Momentum Health for my husband (who is actually rather busy). He happens to be the principal member and I am a dependant, yet I am not authorized to converse with Momentum until my husband signs a consent to disclose information form (their wording) for me as either a third party, a financial adviser or employer representative, of which I am neither... I have asked for clarification going on 4 times now on WHY?!? and in what capacity this needs to be completed, the form also requests an expiry date for the consent - should this also be filled in?

What a joke! So if you're not the principal member you're a non-entity obviously. I hope I don't get second rate health treatment too because I am not authorized correctly to receive proper care!

"Our record reflects that the consent form, which allows you to interact with the Scheme has been previously forwarded to you.

The form has been re-attached, as it is required to be completed by the principal member to authorise you interacting with the Scheme for any future enquiries."
