Mmembership not activated since January 2013

I joined one of the major 4 banks in January 2013. I completed bankmed forms on the 10th January, they were send back because they were incompleted. I completed them and they confirmed receipt on the 17th of January. I wanted to consult with the Doctor couple of times and I was told my membership is not activated. I contacted Tracy she said she will follow up nothing happened, I contacted S Bassier she said she will come back to me, nothing happened, I then contacted J Bason she promised me exactly the same thing nothing happened, I contacted PMABSATEAM, Still no response. My payslip has been debited for Bankmed but I cannot utilize the membership card. Someone in Bankmed please activate membership no 000192740 ASAP
