DISCOVERY adjust debit orders just as they please

Where does this annually income verification thing comes from??? I feel uncomfortable exposing my financial status each year and who knows where and with whom it ends up with. Never the less, after submitting once again the income verification form for 2013 Discovery adjusted my premium as they please and without my consent!!!??? My supposedly premium is R1757 (in Dec 2012), in Jan 2013 increased to R1945, Feb R1945, and in March 2013 increased to R2829???!!! Where does DISCOVERY get this amount??? Who gives DISCOVERY the right to adjust debit order amounts as they please??? I didn't make provision for this amount in my bank, now who are responsible for the unpaid bank fee??? I've received a sms that our KeyCare Core health care plan are now suspended due to non payment. What happens now should myself, my wife or 1 of my 3 kids ends up in hospital???
