Can't cancel my policy

I've been in contact with Old Mutual customer services over the past few months trying to cancel two policies that have matured. At the local Nedbank branch i was helped with all the necessary documentation that was needed, this includes a copy of my ID, bank statements and the cancellation and surrender forms. Forms for both policies were filled in simultaneously and faxed in one fax along with the valid copies. One of the two policies paid out within a few days and I was very happy with the speedy service but now I've been having endless grief trying to get them to pay out the second. I phone once a week asking about the policy and am given the same old story about the form not being sighted in response to which I've the fax sent AGAIN with all the necessary info but still nothing. Every time I call I get promises that the matter will be handled speedily but nothing comes of it. I'm still currently investing with Old Mutual and would like to continue but if I can't even get an answer as to what I can do to speed up the process then I'm lost.
