Policy claim being delayed

My husband passed in October 2011 and he had a investment policy with Old Mutual. I have submitted all the necessary documents eg: notice of death (J294), next of kin affidavit (J192) and tax reference number of the deceased by SARS, as required to make the claim. But with no success of receiving any information from Old Mutual or any pay out from them.
I would like the assistance from Hello Peter on this matter.
