Incompetence to do their job

On Sunday 30 December 2012 I went into the Waterfall Mall branch to change my details, including my address amongst other things. I took my green bar-coded ID and my rent contract to the branch. After talking to a consultant I was told that because my contract does not have an end date that I needed someone to come into the branch with their ID to sign as witness. My mother then came to fill in that documentation. The incompetent consultant could not even record my address correctly off my contract. She wrote it as 103 and not 183. Now my account is blocked because she did not FICA me on that date which was after the date where they must FICA their customers. I spoke to a manager at the branch now who places all the blame on me, stating that there are two forms and I did not fill in the correct one (even though I did) and I am being unreasonable and so on... I cannot go into the branch and need them to fix their mistake. It is not my fault to fix if their consultants cannot do their work.
