Still being charged for paid off mobile device

I paid off my mobile device with FNB in August 2013, up to date i am still paying he insurance charges which have suddenly gone up from R10.00 to R40.00. I am frustraed by how this transaction is being handled by FNB. especially after numrous calls and e-mails to request reversal of these transactions. the "how can we help you" slogan is not working for me.
