Fonesure debits without auth from me

Have a debit orders on my bank statement from a company called Fonesure

Fonesure MD 1404353-2 R62.50 pm

I have never taken out insurance of any sort nor have I signed any debit order authorities for this amount.
I would like to be provided with the following information from this company...

A signed application form
A signed debit order authority
A copy of my contract clearly stating what items i have insured.

This is going off a while now. I spoke to Steven today at Phonesure, he cancelled the debit and said he will send a a request for them to listen to the call. I ALSO WANT A COPY OF THE CALL to confirm if the fault is on my side or not.

If not they need to refund me ALL the money they deducted. I will go thru the bank statements to make sure they don't mess me around.

Only phone call i received was regarding a R100.00 voucher but no one informed me i need to pay for the voucher or for any other extra things.

WHY WILL I TAKE OUT PHONE INSURANCE FOR A PHONE THAT ISNT EVEN WORTH R2000.00 its not like is a i pad or anything.
