Phonebox debit without my auth

I have just come across a debit order on my bank statement from a company called PhoneBox

PhoneBox MD 1404353-1

I have never taken out anything of any sort nor have I signed any debit order authorities for this amount.
I would like to be provided with the following information from this company...

A signed application form
A signed debit order authority
A copy of my contract clearly stating what items i have insured.

I spoke to Cathy today regarding this. She cancelled this debit and said will send a request so they can listen to the call record. I ALSO WANT A COPY OF THE TAPE. Can only after that say if a refund will be done.

Bull k i am sick of this. Since i took out a contract with Virgen Mobile all this below things was loaded on my account without me agreeing.

Going to check my bank statements to make sure how much they took over the last months and i want ALL my money refunded to me.
