Unlawful debit and Zero feedback

I made enquiry regarding a loan in January however did not accept quote amd declined all and sundry. In July I noticed an amoutt being deducted from my bank account by Direct Axis in the reference. I began enquiries and was sent on to Hollard as being responsible for deductions. This was confusing as I had no documentation, where does Hollard A THIRD PARTY come into play. Mellisa in Disputes dept then assisted saying she would investigate etc. I heard no more until Hollard sms'd me to advise my policy is in arrears... WHAT policy.
Direct Axis has not responded to my many emails and telephone calls and the number sent on the sms by Hollard just rings... very worrying.
I want clarity in this matter immediatly as to how and why this could happen and a full refund including all bank charges.
