Can't do your job properly???

My father works in the Congo, with very limited and very slow internet access and very limited phone access.
He was sent an email on the 8th August to complete a "SSI form" for his "cross border payments". He completed the form and sent it back.
He then sent another email on the 15th August asking whether the completed form is correct.
Then Percy Mavhungu said in an email, on the 21st August that "HI EXCON, The form Is fit, please proceed "
On the 22nd August he was told by Gerard Voskuil "This is not the correct SSI form for the client to complete."

WHY on earth would you send the client an incorrect form to complete in the first place??? Is it really that simply to just attach the correct form that YOU want completed, on the email that YOU sent to the client?
