No help for domestic worker - insist on wrong form

My domestic worker, Jacobeth Monye, lost her ATM card in December 2012 and needs a replacement card to access her bank account. Every time she goes to her nearest branch - Key West - she is given a form that needs to be completed by an employee whose address she shares. She rents her own accommodation and doesn't live-in with any of her employers!!! The stupid staff at the branch insists on having the form "Declaration of Residential Address by Employer where Employee resides on Employer's Property" completed. I've even written a note on the form with my contact details for the staff to give her the correct form, without any luck! I do not walk around with cash and also don't expect her to. Please can someone from Absa assist us in the correct professional manner that every client actually deserves, even though I know that it is a tough call for Absa.
