FICA Block due to BANK fault

I produced all FICA documents (for a TRUST) to the bank. They insisted that I do it again, as they lost/misplaced the documents. After 3 months, the block was lifter, but for some reason unknown to me, they blocked it again, without ANY communication. In phoning the call centre, I was told they are unable to assist me and I have to go to a branch with all my FICA documents AGAIN
