ABSA BLOCKED ACCOUNT BECAUSE OF FICA... again

ABSA blocked my account because they claim we did not do the FICA, my problem is how many times must we do a FICA, 3, 4 or maybe 6 before they update their system.

I went to the bank and they just reset something and we could work again, without any FICA documents.

How many time is this gonna happen because this is not the first time?
and
Who is responsible for this because I need someone to phone that can take the heat because this has cost me money this morning.
