Cellphones not insured when there was a request

I took out two phones at the Vodashop in Cresta, first phone was in December 2011 (Consultant Jose), and the second phone was somewhere around July 2012 (Consultant - Enslin), On both occassions I requested that the cellphones be insured and signed all the relevant documents to insure the phones. To my surprise when I had a bit of a problem with my phone and took it to the store on the 12th Jan 2013, I was told that the phones are not insured and the reason I was given was that the consultants didnt submit the information to the insurance company.

I made a call to the store and spoke to the assistant manager Jason Elliott who went to the extent of telling me that the store cannot be held liable for the incompetency of the consultants, and that the store will not be buying anyone a new phone (which I never asked for, I just needed to know what one does when things are as they are). In addition to this the "Assistant Store Manager" says to me I didnt sign the contract with the store but with the consultant

It's shocking that the Vodashop in Cresta has two incompetent employees (add Manager) and the store can not be held liable for their incompetency.
