Unacceptable customer service

I have contract with mtn for a year now. i notified them that I'm going to do cash payments and that they should only deduct the insurance from my bank account, which i gave them in the branch. a month or 2 later i saw on my bank account that they never deducted, went to the branch and told them the prob & they "rectified" the problem. following month the same problem accured and no one could give me a valid reason, i gave them my salary date and my bank statement, this has been acquiring for the past few months, and all i do is fill in insurance forms! beginning of march i was at the bottom mtn branch in parow to send my cellphone away, when they notified me that i must get my cellphone, the lady told me that the cellphone could not be fixed! on the 23rd of march i was at the top branch again to explain my problem and the consultant told me if that happened already than there is nothing she can do, and she told me to go to a local cellphone shop, where i had to give out a R250.00! which is unacceptable! i asked them if i can get another cellphone with the same package & the lady said no! but they are allowed, coz blackberry 9300 is disconnected world wide! what do i do? ple
