Unacceptable service

I have been trying to reactivate my sim card for the past month now. I went to Vodacom Middelburg Mall and the consultant gave me a form to complete and told me I can send it myself to the e-mail address and fax number on th form. I also then purchased a new simcard.

After I sent it in I waited almost two weeks and no reactivation happened. When I phoned their call centre they informed me that I did not sign at the T & A's. Now the T & A's are on the same form which I signed and no employee from Vodacom informed me that I should sing twice on the form. I then signed it and send it again. I waited three days and then phoned their call centre again only to be told I must send a copy of the simcard. On both occasions I spoke to a consultant with the name, Stella. I then send a copy of the simcard as well. Up to date I have had NO response and my number is still not activated.
