Extra Contract

In Nov last year, I applied for a contract with Mtn. To start with- the lady in the store didn't listen to me when I told her that I already had a Mtn sim. I did not understand why I received a new sim card. After a week my new sim still did not work- I was in the store 3x times that week, every time I was told the system is slow. The fourth time in the store the lady realized I had an Mtn sim not Vodacom, after I've told and showed her the sim several times. On my statement the end of the month, double the amount was subtracted from my debit order. I phoned the customer line and the mtn branch, both said it is start-up and extra costs because the contract wasn't started at the beginning of the month. Second statement, it happened again. I phoned both the branch and the customer line again. The branch did not get back to me as promised. I then found out, that somehow, there is an extra contract on my name! Which I have neither applied for, nor signed for! It appeared that the lady in the store magically applied for 2 contracts, apparently sent a request for 1 to be cancelled but never checked up. Isn't this more or less like? Now I'm struggling to get my money or an answer!?
