STILL NO ITC numbe

I dont know what the i have to do to get an itc number.
Story:
12/10/2013 Midday my phone gets.
12/10/2013 afternoon in MTN store, Give them IMEI number. Donna says your sim has been blocked and your phone blacklisted.
13/10/2013 afternoon Find out my sim card is still active and still been used by the.
Phone 808 get sim card blocked after 20min.
16/10/2013 go back to MTN store give IMEI no and letter head to another staff member.
29/10/2013 receive blacklist request form. Fill it in and faxed it back. later get sms saying they need letter head and signature for permission to black list.
Phoned 808 29/10/2013 at 5:00pm want to speak to manager, lady on the phone wont put me through, then after 20min shes says the manager is busy he will phone you back.
At 7:30pm 29/10/2013 he phones. That is not acceptble. 2-30 hours to phone back.
NEARLY after 3 weeks of my phone getting. I have sent in form after form, letter head after letter head.
STILL NO ITC number what the is going on at MTN.
I cant claim from insurance because of this.
I WANT it FIXED TODAY!!!
OR i will contact my lawyer.
