Upgrade offered new phone

Dear Vodacom

I have been with Vodacom for the past 6 to 7 years with no issues.
My current nightmare with Vodacom started when I upgrading online, Vodacom gave me a new contract 0761237709 instead of upgrading my current line. I immediately contacted Vodacom (useless) call centre, I was promised that they will contact RAM to collect the phone and cancel contract.
Then the call centre agent offered to upgrade the contract that was due, it was done and delivered the next day. But since then 09-10-2013 I have been waiting for someone collect the phone. instead Vodacom decide to debit my account. I have been in contacted with Vodacom call centre they keep on transferring me from one consultant to the other who keeps on promising that my query will be solved shortly. Can someone from Vodacom help a cell phone is a risk it might get lost who will be responsible?
I also want Vodacom to reimburse me for the debit that was rudely deducted from my bank account including bank charges. Vodacom is useless if query is not solved I will have to cancel all my contact, I will also make sure that all my business partners.
