Got ITC'd unfai

I got a contract in August 2012. With the signing of my contract, I signed a debit form. It was MTN's fault, because the first three months they debited my contract from someone else bank account, then it put me in arrears. I have faxed through 4 debit order forms already, and yesterday I had to phone to hear that I still have a cash account. Now that my account is up to date, they can not give me a letter for my bank to say that it is up to date for the ITC. This was their fault from the beginning, now my bank does not want to help me because of their fault. THIS IS POOR SERVICE!!! I want my ITC record corrected!! If they did things right from the beginning, I would not have missed payments.
