The poor service lost me money!

I went into Vodashop N1 City on 16 September 2013 requesting my Blackberry, that was with my car, to be blacklisted as i need an ITC number to claim from my insurance that i have at Edgars on this phone. The ITC number has to be at Edgars within 48 hours to claim from the insurance. The staff at the Vodashop was not helpful and they did not seem to know what to do about this simple request as i had to deal with 3 diffrent people before a lady said that she would e-mail a form but i would only receive the number in 72 hours and she gave me the proof of delivery stating that she sent it to Tigervalley Vodashop. The manager at Edgars N1 City helped me and attempted to get the number from the call center and got no joy and Vodashop N1 City still has no ITC number and just claims ignorance which is now causing my insurance to go to waste and me to sit without a phone.
