Not doing there Job

I needed to claim from cellphone insurance and had filled the claim form out at the store, This was the 14th of July 2013.I was told that the form would be sent of the next day. On the 18th of July having no response from the insurance department i called the call center. I was advised that they have not received the claim form. I then went to the store on the 20th July to esquire as to why the form had not been sent. I was told that they will send it the same day. I have called the call center today (23rd July 2013) and still they have not received the claim form.
THIS IS TOTALLY UNACCEPTABLE.
