Fail to give me a statement but demanding payment

I had a cell phone contract which i started paying in October 2007 the amount of R145.00 by debit order. Vodacom stop the deduction and in September 2009 I received a call from them that I owe them R5000. I arranged to pay R600 per debit order from October 2009 untill October 2010.Vodacom stopped the deduction again. According to me the R5000 was paid up seeing that they has stopped to deduct the money. December 2012 I received a call that Vodacom has handed me over to VVM Debt Collectors. I was supprised because I never received any notification that I still owe them. I asked for a detailed statement in order to see how much the phone cost and how much I really owe them and they just refuse to give me a statement. My residential address are also incorrect because I never stayed in Middelburg. All I want is a statement and how can I pay if they refuse to give me a statement?
