Account Settled and an unauthorized purchase

So I settled my YDE account October 1st, 2012. I received a statement confirming this the end of October.

In November, I was surprised to see that YDE is sending me another statement. So I opened it and it had a purchase of R960.00 that took place at Cavendish Square (this was the unauthorized purchase). I immediately called Customer Support, and they advised me what I needed to do, which was obtain an affidavit from the Police and fax it to them with a copy of my ID, which I did. In December, I received another statement stating that my balance is now overdue. I then called them and faxed through the same information. Which I thought at the time it had been settled.

I received another statement yesterday with the same information stating that it's overdue, and I called them and I have to fax this information through again. What is the point of having to fax something through again? I mean honestly, I am just so fed up and frustrated, and don't forget I have been black listed because of this issue. What should I do to finally lay this issue to rest? I'm just so tired of fighting this with associates that are unable to fix the issue or point me into the right direction.
