Struggling to get paid up account closed

I've been struggling for the past seven months to close this account as it has been paid up, I had to pay additional service fees due to the account not being closed. to date I am still receiving statements and sms's about the account. This really frustrates me and it also makes me feel embarrassed. I filled in forms and all kinds of staff but still no luck.
