Cutting of service when they did not debit account

I have a problem with TopTv (starsat). I have phoned them in November 2013 to active my debit order for the end of December but the debit order never went of so I phoned them again on the 2 January 2014 the person I spoke to said that it was never activated so he activated it and said that it will be deducted in the next 24-48 hours but that never happened. So on 15 January they cut my service when I phoned the evening no one could give me straight answers of why the debit order did not go off they also said that the system shows they tried to debit it the 18th of December after I stipulated it for the 1st of each month but on my bank statement it does not show any returned debit order from them. I spoke to a supervisor that said that he will speak to accounts department and he will phone me on 16 January but he never phoned. It is not my fault they did not debit my account but now I am being penalized for it.
