Waitng 2 months for UIF FORM

I was laid off at my job in May 2013, due to the strikes in Rustenburg, I wanted to apply for unemployment. I went to Allied at told them they never gave me my unemployment form when I resigned by them in Aug 2012. I am unable to claim unemployment because, 2 months later, I am still waiting for the form from Allied. I have phoned numerous times, spoke to Jessica and she told me she is waitng for their head office.
