Double deduction without consulting/advising

In the first week of February my wife submitted an application for medical aid for myself and three kids and on the 08 February upon requesting a Medical Certificate we received a medical certificate that showed it was just me as the member and my wife as the principle member, after querying it with Bank med, they went back to fix the problem and my wife was told the application is pending, we were under the impression pending means we are not to use or do anything with the medical aid until the problem has been rectified. No feedback or correspondence was received between the 08 February until this week when we received the Medical certificate reflecting all the members correctly, however Bankmed went and deducted double installments for February and now for March 2013, without first consulting and secondly explaining why we need to pay double installments. I want my one installment to be refunded in full since the error and mistake was made on their side, not capturing and including all the members the first time around and pending our medical aid, why would one have to pay for something you didn't have?
