Unauthorized triple debit

I added my wife back to my medical aid effective 01/09/2013 (1 Sept).
The documentation was only submitted at the end of September, so they would have to double debit me on my wife's portion on the 1st of October to have it effective from 1 September.
I have no problem with this.
What I have a problem with is the fact that they forgot to do this on October. I checked my bank account this month and I saw that I am missing 2000 rand somewhere. So when I finished digging I came to the conclusion that my medical aid bill was about 2000 more than the bill was supposed to be for the 1st of November.
I then called momentun to question this massive amount. They told me they forgot to debit me in October, so they had to triple debit me the 1st of November.

This is unacceptable!! I want to know where on black and white I signed that they are allowed to triple debit me if they "forget" to do it.

I have not made any claims on my wife's name as yet.
I expect a full refund for September, October and November with my wife only being active from the 1st of December 2013.
