On treatment date 04/05/2011, replacement staff was used in my practise for that day, as I attended a funeral.
The pasient was seposed to get a authorisation nr. for the treatment, the patient did not get the autho and the replacement worker let the pasient saw Dr Coetzer without a autho, Dr did a Yag Laser where the lenses get cleaned in the rooms the account outstanding came to: R3311.30. The next day I phoned the m/aid straight away to get the autho nr. they refuse to give me a autho nr as for them that was "a late autho". If got ref nr's fax delivered reports I also e-mailed all my documents to the scheme manager. I also wrote a motivation letter Ref: .060511 Q 57425.My colleage Mr P. Bergh also lodged the complained with the Rep that attended members compains in town. She gave Mr. Bergh this Sir's name but was asked not to mention that she gave it to him. Mr De Sousa.
