List of Network GPs and Dentists

I have been trying to get a list of Network GP's and Dentists from Affinity for over a month now. Four calls to the contact centre. Every time I am told that they will send me this list via email. My email address is confirmed, and then we say our goodbyes and the call is terminated. Two days later I either get an email wanting to sell me a product that I already have (health insurance, from the service provider) or I get an email with my "Welcome Pack" in it. I have read this welcome pack cover to cover, including the 2 welcome packs I have received via snailmail. It contains no information pertaining to who my GP and or Dentist is supposed to be. Just a reference to "out of network" providers. No further emails anywhere... They have my correct email address because I get the spam from them and the welcome packs. Made a call this morning and to my surprise the person wanted to transfer me to the MD, who was not available to take my call, but a promise from the PA / receptionist that "he will call you back, sir... please give me your number? "
