Handed over to ITC by them

I was in hospital in November 2012 and my doctor had some blood tests done. My medical aid short paid the account with an amount of R378.50. My account was only printed on 17.05.2013 and sent to me. I made arrangement to pay it of in 2 installments. I paid my 1st installment on 21.05.2013 for an amount of R200.00. The final installment for R178.50 I paid on 20.06.2013.
On 17.07.2013 I received a message from them saying that the have handed me over to ITC?! When I phoned them I was told that they did receive both payments but could not allocate the R200.00 to my account unless I sent proof of payment. The payments were made via EFT and I stated my name and invoice number on the payments!
I have been handed over to ITC due to the unallocated amount of R200.00!!!
I want them to remove me from ITC with immediate effect.
