Incorrectly assesed

I had submitted the pre assessment request plus a dummy account on the 16th July 2013. This is what discovery suggested I send. However now they have misread the details and have declined the request because of an error. it is in fact MRS ANSTADT and not MR A. Please contact us ASAP in order to resolve this problem as I do not have time to wait another 5 days for me to re submit-this is actually a threatening emergency procedure and they are messing around by not reading forms correctly.
