Trying for 3 months to get something corrected

I have been trying for 3 months to get a mistake corrected that a Discovery staff member caused. I've had a health check at Clicks on 8 April but Discovery never logged it on my records. I received the Vitality points for it, but Medsaver and Chronicare still show "No Health Check". This means I'm losing 15% cashback on all my purchases, since April. Furthermore Discovery linked the WRONG Dischem branch (Verdi) on Chronicare. It should be Raslouw Dischem. I've written dozens of times, and phoned, but have had no action. I even received an email stating it was corrected, yet on my profile it still does not show.
