Once agian authorization approved & claim not paid

This is the second time ive asked my detist to get auth before my appointment in 2012 and then after a month i get a statement to state that the medical aid hasnt paid the full amount and i wasnt even aware of this my last appointment was in dec 2012 and then 3 months later i got a call from the dentist to say that i owe them money, i spoken to a lady at discovery i can not recall her name but she said she will check and and sort it out for me and then today i got another call from them saying if i do not pay they will have to give the matters to legal dept... authorization number 2012112715400004 stating APPROVED and clearly not, im very unhappy as this is the second time for the same reason...
