Refusal to pay for blood test / failure to arrange

You are refusing to pay for a cholestrol blood test. Your documentation states that PRE-NOTIFICATION IS NOT REQUIRED for Blood pressure test, choletrol and blood sugar test. I obtained pre-authorisation for ALL other tests. ALSO The cholestrol test is COVERED if Health Assessment results indicate a total cholestrol of 6mmol/L My blood test results which I have sent to you shows 7.04mmol/L and the test is therefore PAYABLE. As a lawyer/attorney I find your company's attitude alarming.
Following my health assessment via GP he prescribed medicine for HIGH BLOOD PRESSURE and HIGH CHOLESTROL. As my chronic provider is the STATE I was sent a State Chronic Benefit Application form and a list of Chronic State Hospitals. I visited the Victoria Hospital on Friday 31 May I showed Main Reception my form and they took me to A & E and they said they didn't complete any forms for Medical aid/ and they were not obliging to complete the form. I spoke to RUSHINKA in your Exec dept on 31/05 and she advised me to send her an email giving those details which I did on 3 & 6 Jun together with prescriptions and blood test results. STILL NO RESONSE
