Annuity option form

I have a maximum income living annuity with OM. I wish to increase my drawdown to maximum for the year. OM refuse stating that they never received the annuity option form before the last anniversary. I NEVER RECEIVED THIS FORM. This is a serious matter for me. I am a widow, over 81 years old and in poor health and need every cent.
