Process debit order which cause bank charges

Integrisure was asked to change policy charges from debit order to cash like the rest of my policy @ begin of Jan'13. Istructions were not followed so the bank charged me a fee of R138 where as the policy is only R56. Due to the neglience of this insurance group, the unnecessary charges were made. Who stands responsible for this unnecessary charges???
