Debit Order on a wrong date

1.Debit Order goes off on the 14/12/2012 and 18/01/2013: I wanna know who advised African Bank to debit my account whenever they wish to and who will pay for the late charges and returned debit order charge
2.I gave African Bank permission to debit my account on the25th, today it's the 4th of the next month and debit order still hasn't gone off.
I took the initiative of requesting for a debit order to be reloaded sent email to Sihle Masilela [Email Removed] the 25th and on the 28th January she replied and requested for my Id number I am still awaiting for her to get back to me.
3.On Saturday 2nd February I receive a call from collections department, a rude lady asking me why did my debit order return unpaid on the 18th
