Unity hospital Cash Plan insurance

I have taken a unity Hospital Plan with standard which i have it for quite some time now, what came to my surprise i was hospitalize last year which I spend about four days in hospital after i was discharge, I have made a claim and they send me a form to complete of which that was done and on the form was a part that was supposed to be completed by the Family Dr which the Dr who Treated me in hospital completed that part and lee-aan contacted me that to actually have that rectified and also requested a copy of the hospital account which of that was rectified and my Family Dr completed that fax back to her and lee-aan even requested additional information from my family doctor which that was prepared and send back to her, she called me in October to tell me that she has received all the information that my claim will be send for processing, but till to date my claim has not been paid, but premium goes through every month and when I call their call center to check the status of my claim, I've been told that there is information still missing and lee-aan no longer working for them, they will investigate and get back to me but no correspondence has been made eversince.
